This is a great opportunity for a Health Start Specialist to join our agency. This position is a community based position. In this position, the individual will educate, support and advocate for at-risk families. The Health Start Specialist will promote optimal use of community based family health care services and education services. The Health Start Specialist will conduct home visits for pregnant women and/or newborn infants through the age of 24 months and attend monthly staffing with Social Worker and Community Nurse. The Health Start Specialist will advise client or community groups to ensure parents understand childhood immunizations and provide resources to include immunizations. The Specialist will also assist families to apply for community services, including Medicaid and Women Infants, and Children (WIC). 

Skills / Requirements

High School Diploma or GED.
Minimum of 1 year of experience working with children and families.
Extensive knowledge of community resources and services available to assist children and families served.
Strong communication, presentation, and group facilitation skills.
Current valid fingerprint clearance card or meet eligibility requirements to obtain one.
Position may require individual to attend meetings, training, and agency activities and events outside of regularly schedule work hours.
Must possess valid AZ Driver's license with no major infractions for the past 39 months. 
Maintain appropriate vehicle liability insurance as position will require use of own personal vehicle.
