Family Support Partner - Apache Junction

This position is full time and must have the flexibility to work an occasional weekend day or evening to meet a client's needs.
The Family Support Partner models the skills necessary to maneuver within the behavioral health system and other community programs, thus serving as a system guide and advocate for the parent. Provide direct support to families as an active member of the Child and Family Team. Encourage family members to move toward self-advocacy in the attainment of their own, culturally specific life goals for themselves and their children and helps them develop the skills to do so. Work with the family to gather information regarding their strengths and needs, as well as provides information, which enables the family to make decisions in the child and family planning process and beyond. Provide information and connections to community resources, and helps the family navigate through the processes of the education, behavioral health, juvenile justice, and social services systems.
Skills / Requirements
- Accept referral, make initial contact and orient the family to the Child and Family Team process and relationship to the Arizona Vision and 12 Principles. In preparation for the child and family team meeting, work intensively with the family to identify formal and informal supports, invite them to the meeting or a family meeting prior to the child and family team meeting. Assist the family in identifying its strengths, family culture and concerns to be presented at the child and family team meeting.
- Deliver 40 hours of work per week, with a goal of 30 hours direct client contact, which includes case management as needed. Productivity standards established at a minimum of once a year and as needed due to contract changes. Complete all tasks agreed upon by timelines given.
- In collaboration with each child and family, support services that meet the following 12 Arizona Principles: Collaboration with the child and family; Emphasis on achieving functional outcomes; Collaboration with others, including participation in unified assessment, planning and service approaches when children and their families are involved with multiple systems; Access for children and their families to a comprehensive array of sufficient behavioral health services to meet their needs; Best practices; Most appropriate setting; Timeliness; Services tailored to the child and their family. Stability in placements; Respect for the child and their family’s unique cultural heritage; Independence, and; Connection to natural supports.
- Support the family in expressing their perspective at team meetings while developing a multiagency team plan by building relationships and working together with other agency stakeholders. Monitor implementation of the team plan from the family perspective and progress towards goals. Participate with the team in revising as needed.
- Locate and orient the family to additional informal supports available in the community and help them connect with them. Provide assistance in completing applications for community resources as needed. Participate with team members and agency staff in identifying and removing barriers within the agency or across agencies that get in the way of family-driven teams and family-centered, strengths-based, community-based and culturally competent services for children.
- Attend regular scheduled weekly supervision. Seek out additional supervision as needed.
- Minimum of a High School Diploma
- Experience as a parent, foster parent, adoptive parent or a caretaker of a CHILD with Behavioral Health needs and experience with the children's services systems. The child MUST HAVE LIVED WITH YOU.
- Must have received behavioral health care services at some point current or past.
- Be very organized and can manage their time effectively in this busy position
- Be computer literate, good customer service skills, basic typing,
- Able to work professionally and effectively with others from diverse ethnic and cultural groups
- Good written and verbal communication skills
- Possess a valid ARIZONA driver's license w/no major infractions for past 39 months
- Must be eligible to obtain a Fingerprint Clearance Card or have one already
- Bi-lingual (Spanish) preferred
Additional Requirements and Responsibilities:
- Perform those assignments agreed to in the team meeting and defined in job description, and provide on-going support by being there to listen or help them in communicating what is and isn't working. Also at the family’s request and when appropriate, meet with system staff and
- supervisors to continuously improve quality of processes.
- Attend other meetings such as IEP, ISP, behavioral health or probation meetings at the request of the family.
- Participate in identifying training needs of Family Support Partners and co-workers in applying the 12 Principles and AZ Practice Model.
- Maintain a professional demeanor and positive working relationships and effective ongoing communication with all referring organizations, behavioral health clients, parole officers, case managers, service coordinators and school personnel.
- Position requires driving of company van and/or personal vehicle. Must maintain a valid Arizona driver's license at all times.
- Evening and weekend hours may be required to accommodate the schedules of children and their families.
- Attend meetings and trainings as requested.
- Demonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender or sexual orientation.
- Maintain professional conduct consistent with ethical professional practice and the policies and procedures of the agency.
- Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners.
- If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required.
